SASA Dance Studio – Waiver, Release & Consent
I, the undersigned participant (or parent/guardian if under 18), understand that dance training and the associated physical exertion involve certain risks of injury. By signing this waiver, I voluntarily agree to assume all risks and responsibility for any injury or damages that may occur during classes, rehearsals, or performances organized by SASA Dance Studio. I will abide by the Studio rules - No street Shoes and unregistered kids
I hereby release and hold harmless SASA Dance Studio, its owners, instructors, and staff from any and all liability, claims, or causes of action arising out of my participation in studio activities. I also confirm that I am in good physical health and have no medical conditions that would prevent my safe participation in dance classes.
Furthermore, I grant SASA Dance Studio permission to use photographs or videos of participant(s) for promotional purposes, including social media and the studio website. I have read this release and fully understand its terms.
I have read this Waiver, Release & Consent carefully, understand its contents, and agree that by registering and/or clicking “I Agree” during online registration, I am signing this document electronically and that it is legally binding. If I am registering a minor, I confirm that I am the child’s parent or legal guardian and have authority to agree to these terms on their behalf.
